    For starters in my practice I knew about the disinhibition correlation with Parkinson’s.Never saw it in couples.Most of them slept in separate beds for convenience reasons turning around from side to side.I had a case of a pt who was stalking me a bit,bringing me gifts and flirting.I don’t know about gambling which is supposed to be the other problems

    As it has been said it is a tricky situation.First thing I advise my pts is sleep in separate beds.That provides better sleep for the parkinsonian.Secondly there are ppl with genetic predisposition who decide before even the onset of the disease to go ground floor.Of course that is not always an option for everybody.Third there are apparatus seating facilities attached to the stairs that can help the person to get to first floor.Depending on the house and durability u have to ask first an interior designer or engineer if its going to last and second the cost.Then there are the bathing problems.Showers are indicated not a bath tab.These problems are the same with ppl with multiple sclerosis or stroke.Thanks for listening.

    This is not as uncommon as u think.The critical thing is the diagnosis.Is it parkinson’s for both of u or one has parkinsonism,which means that many small unidentified strokes might present with similar symptoms?To ensure that,u must both have a dat scan positive.In my practice women get it harder since after menopause there are bone problems and need constant attention,also urinary incontinence might be more common if she has given birth.Talk about all this with your neurologist

    I am afraid that u got DBS a little too late.It does save the day the first 10 yrs for a couple of good yrs,depending on the early beginning of diagnosis.21 yrs with PD means another 5,altogether 26 yrs.This is medical information from me-a neurologist.As u correctly say it might do wonders for many yrs with the tremor and it is used in extreme cases of idiopathic tremor.In my country in Europe many immigrants sold their home to get DBS and nothing happened.They carried bedriden their loved ones home.Partly it is the fault of the neurosurgeon who think they can cure any kind of neurological disease from multiple sclerosis to epilepsy to Parkinsons and so forth.

    It has been proven that prior to the onset of the disease the “pt” has sleeping problems.Napping is due to the disease,not enough dopamine,like the off phenomeno.We call it non-motor symptoms of Parkinsonism.It may accompany low blood pressure or it may not.What I have seen as a neurologist is that u ve got to check the heart,not focus on fatigue and kinetic symptoms only.Many sudden arrests happen out of the blue.Best of luck and take care with that heat which is never on our side in Neurology

    Weight gain is a complicated issue in Parkinsons.U have to consider that dysautonomia has a correlation which means that the gut and all the organs are in slow motion with the disease,so indigestion,reflux and other stomach issues are presented-u all know constipation.Nevertheless there is another important issue which is malfunction of certain brain grey cells that lead to overeating or other aberration issues like gambling or overspending.If u want to eat more think hard about that.Its the disease,like binge eating.Thank u and since its summer and high temps,plz dont eat.A professor of mine once said,eat in a small plate like your cat’s.

    The difficulty of the disease is in swallowing liquids,as there is a risk of aspiration pneumonia.The solid foods not so much but there is a chance that it might happen.U can try putting his food in a blender and feed his slowly.Usually these situations happen when the pt loses his mobility and is bedriden.If he walks it is more difficult to choke.

    It is interesting to hear opinions about any drug for Parkinson’s as a neurologist.Clonazepam belongs to benzo’s which kinda are contraindicated for parkinson’s since they are muscle relaxants too and clonazepam lasts about 20 hours.It is used in anxiety disorders more than once a day but the dose u all mention is small 0,5 mgs.Trazodone is a drug that we use often an antidepressant with sleep properties that came out in the 70’s.My only suggestion is that if u use it more than a year,do a cardiac echo cause its a tricyclic antidepressant and it might mess with the cardiac valves.Keep strong.I agree with one participant that u must have a stock of clonazepam,since its a cheap drug and it vanishes although in Europe there are generics.It will never run out completely,since we use it in Epileptic children too.Thanks

  • Recent studies for instructions of hydrations declare that 5 glasses a day are enough,lets say 6 for summer.I dont think that there is a difference for Parkinson’s.Having autonomic dysfunction too,it is difficult for excess water being passed.Anyway the difficulty swallowing water is a symptom of the disease well known.

    U have to be careful with salads like greens,cause they cause the muscles are tired out and the pt might choke.There are two stages of the disease the difficulty in swallowing both hard and liquid foods at first and then the second mostly.My pts understand on their own (caregivers mostly) to put half of the food in blender.Not all the pts have trouble with smell and taste.My opinion is that small meals,depending and on the drug doses are ok.Second point is the weight gain.It plays a very important part,since the movement is limited so try not to push if disinhibition is also present.I don’t recommend sweets like chocolate or candies (aforementioned by another participant).Levodopa malabsorption in the intestine has nothing to do with food absorption-another mechanism.

    Stalevo who is levodopa with the comt enzyme is much more powerful since it helps levodopa reach the brain.High doses are recommended up to 800 mg as time progresses,unless we have dyskinesias

    Dear Kathy.PSP belongs to the former title of parkinson-plus syndromes.Its very rare and easily spotted by vertical eye movements disarray.In PSP levodopa does not work at all.After all these years and multiple metanalysis the most common form of atypical parkinsonian syndromes is multiple system atrophy or MSA.All organs collapse in time and the person is bedriden eventually.Death is by heart arrest

    Mannitol is used also as a water retention fighter in IV infusion in strokes.Is it in a form of a pill?What is the releasing company?

  • It is interesting-the concept but then u are a lawyer.Any inbalance in electrolytes may mess up with the equillibrium of the brain.J F Kennedy had the same problem but never showed parkinsonian symptoms

    All neurological diseases degenerative or otherwise like multiple sclerosis are getting worse with the heat.Its the dysregulation of temperature in the brain.Try keeping cool.Personally as a doctor,I never change my treatment regime in the summer.
